Meet Our Team

Patient Choice Medical Care's physicians practice general internal medicine, and as such handle the broad and comprehensive spectrum of illnesses that affect adults. They are commonly referred to as general internists.

Prior to going to Medical school Dr. DeSilva attended Oklahoma State University and obtained his Bachelors and Master degrees in Biochemistry and Molecular Biology. He then attended St George’s University School of Medicine and graduated with Honors. He completed his residency training at Yale affiliated Norwalk Hospital in Internal Medicine. 


He is also Certified Medical Director from the Society for Post-Acute and Long-Term care medicine and is the Medical Director of a number of area skilled nursing facilities. 

In addition, Dr. DeSilva is an assistant clinical professor of medicine at Quinnipiac University School of Medicine. 


His hospital affiliations include Milford Hospital and Yale hospital Systems. Currently his practice is in Hamden CT and he resides in Stratford. He is married and has two children. 

Dr. Sunita Chhabra, M.D. is a medical graduate of Dr S.N. Medical College, Jodhpur in India. After finishing her medical degree, she completed her residency and post-graduation in OB/GYN in India. Since moving to the US, She developed and interest in Internal medicine and finished her internship and Residency in Internal Medicine from Long Island College Hospital (SUNY Downstate, Brooklyn). Dr Chhabra then worked in New York for 5 years, before moving to Connecticut in 2005. Since then she has been in private practice in North Haven for 14 years. 


Dr Chhabra is Board Certified in Internal Medicine. She has served on committees of Connecticut State Medical Society from 2006. Dr Chhabra is also attending physician at a few skilled nursing facilities in the area with an interest in geriatric care.



Pratima Patil, APRN received her Bachelors of Science in Nursing from SUNY Polytechnic Institute, Utica NY in 2012 and her Masters in Family Nurse Practitioner from PACE University, Pleasantville, NY in 2016. Pratima has worked in an inpatient setting as a RN for 3 years and has been working in a primary care setting as a nurse practitioner since 2016.

She likes to spend time explaining disease conditions, treatment plans, outcomes and goals with her patients. Primary care is her passion and believes that preventing and managing disease conditions before they compromise quality of life is the key. 


Megan Lipford, AGPCNP-BC, received her Bachelor of Science in Nursing from Palm Beach Atlantic University, West Palm Beach, FL in 2019 and her Masters in Adult Gerontology Nurse Practitioner from Quinnipiac University in 2022. She also received her Bachelor of Science in Public Heatlh from Rutgers University, New Brunswick, NJ in 2014. 


Megan has worked at all levels in medicine, starting with her experience working as a Medical Assistant/Phlebotomist prior to bedside nursing. She worked for Yale New Haven Hospital and Midstate Medical Center specializing in medical-surgical nursing and neurology. She also has inpatient and outpatient experience working in nephrology as a nurse practitioner. Megan has a passion for primary care and building long-term relationships with patients. Outside of work, Megan enjoys spending time with her husband, daughter, dogs, chickens, and goats on her farm.

What is an internist?

Many internists enter into practice following completion of their basic internal medicine training. General internists are recognized as experts in diagnosis, in the treatment of chronic illness and in health promotion and disease prevention, as they are not limited to one type of medical problem or organ system. 

 

General internists receive training that uniquely qualifies them to practice primary care and follow patients over the duration of their adult lives; they often establish long and rewarding personal relationships with their patients. Yet although internists may act as primary care physicians, they are not "family physicians," "family practitioners" or "general practitioners."


Where we practice

Primary care physicians are able to practice in a variety of settings. Dr. DeSilva continues to follow patients while at the hospital, providing continuity of care to their patients. The office uses Electronic Health Records and encourages patient engagement and participation in the patient's care.

 

The use of "My Chart" allows access to patients' medical information, including their medical history and labs, which permits better communication with the office and the clinicians. Additionally, the practice is undergoing the credentialing process for Patient Centered Medical Home.
