General internists are physicians trained to diagnose and treat a wide range of adult illnesses. Dr. Anura DeSilva, MD
Dr. DeSilva is a board-certified internist with a strong foundation in biochemistry and molecular biology. He earned both his bachelor’s and master’s degrees from Oklahoma State University before graduating with honors from St. George’s University School of Medicine. He completed his residency in Internal Medicine at Yale-affiliated Norwalk Hospital.
Dr. DeSilva is a Certified Medical Director through the Society for Post-Acute and Long-Term Care Medicine and serves as Medical Director for several skilled nursing facilities in the area. He is also an assistant clinical professor of medicine at Quinnipiac University School of Medicine.
His hospital affiliations include Milford Hospital and the Yale Hospital System. Dr. DeSilva currently practices in Hamden, CT, and resides in Stratford with his wife and two children.
Dr. Sunita Chhabra, MD
Dr. Chhabra graduated from Dr. S.N. Medical College in Jodhpur, India, where she also completed her residency and postgraduate training in OB/GYN. After relocating to the United States, she shifted her focus to Internal Medicine and completed her residency at Long Island College Hospital (SUNY Downstate, Brooklyn).

She practiced in New York for five years before moving to Connecticut in 2005. Since then, she has served the North Haven community in private practice for over 14 years. Dr. Chhabra is board-certified in Internal Medicine and has been active in the Connecticut State Medical Society since 2006. She also provides care at several skilled nursing facilities, with a special interest in geriatric medicine.
Pratima Patil, APRN
Pratima Patil earned her Bachelor of Science in Nursing from SUNY Polytechnic Institute and her Master’s degree as a Family Nurse Practitioner from PACE University. She began her career in inpatient care and transitioned to primary care in 2016.
Pratima is passionate about educating her patients and believes that understanding disease conditions and treatment plans is key to successful outcomes. Her approach emphasizes prevention and early intervention to maintain quality of life.
Megan Lipford, APRN
Megan Lipford, AGPCNP-BC, holds a Bachelor of Science in Public Health from Rutgers University, a Bachelor of Science in Nursing from Palm Beach Atlantic University, and a Master’s in Adult Gerontology Nurse Practitioner from Quinnipiac University.
Her medical journey includes roles as a medical assistant, phlebotomist, bedside nurse, and nurse practitioner. She has worked in neurology, nephrology, and medical-surgical units at Yale New Haven Hospital and Midstate Medical Center. Megan is passionate about primary care and enjoys building lasting relationships with her patients. Outside of work, she lives on a farm with her husband, daughter, and a lively group of animals.
Where We Practice and How We Support You
Our providers care for patients in a variety of settings, including hospitals and skilled nursing facilities. Dr. DeSilva continues to follow patients during hospital stays, ensuring continuity of care and better outcomes.
In our Hamden office, we use Electronic Health Records (EHR) powered by EPIC, which allows for seamless communication between patients and providers. Through MyChart, patients can access their medical history, lab results, and appointment details, making it easier to stay engaged in their care.

We are currently undergoing the credentialing process to become a Patient-Centered Medical Home, further reinforcing our commitment to coordinated, high-quality care.
