The Patient-Centered Medical Home is a model of care that puts patients at the forefront of care. PCMHs build better relationships between people and their clinical care teams. Research shows that they improve quality, the patient experience and staff satisfaction, while reducing health care costs.
At Patient Choice Medical Care, we use EPIC for electronic health records. A robust system that allows communication with the office much easier with the use of my chart. In addition, we can provide our patients with educational material on a variety of topics in many different languages. Better education and clinician instructions prepares our patients to better understand and self-manage their medical conditions.
Efficient referrals process when needed to specialists allow our patients to receive timely care. We collaborate with multiple specialists in all areas of expertise in the area.

About our practice
Patient Choice Medical Care's physicians are primary care doctors and general internists who are board certified in Internal Medicine and members of the AMA (American Medical Association). They use their skills and experience to provide both primary and specialty care for adults.
For your information, general internists are equipped to deal with whatever problem a patient brings, no matter how common, rare, simple or complex, and are specially trained to solve puzzling diagnostic problems. In addition, internists can handle the treatment of severe chronic illnesses and situations where several different illnesses may strike at the same time.

One way we achieve this is by assembling a team of highly skilled healthcare professionals who are devoted to addressing the individual needs and demands of patients.
Our practice allows patients to communicate with the practice through the use of My Chart. Also using electronic health records allows us to communicate well with the specialists.
Our practitioners and physicians also have access to state-of-the-art technologies that they use in serving patients. Perhaps even more important, every member of our staff does their part to create a warm and caring environment for the people who come to us for health care.
